Challenge

US Payor’s Value Based Care division seeks a scalable solution to identify high-risk patients for heart failure months in advance. They aimed to empower clinicians with actionable insights to design timely interventions, reduce healthcare costs, and improve patient outcomes.

For our Payor client, HF and related conditions like hypertension (HTN), coronary artery disease (CAD), and diabetes (DM) represented a shrunk population driving a disproportionate share of healthcare costs, identifying high-risk patients months in advance is essential to improve outcomes.

Our Approach

We developed a predictive solution leveraging Vantage’s advanced analytics capabilities to predict the onset of heart failure 6 months in advance with high accuracy and scalability. We utilized advanced statistical techniques for robust model development: Feature were identified to ensure the model was well-validated and accurate. For California, the PoC demonstrated potential cost savings of $5Millon if preventive measures were applied to just a fraction of the high-risk population.

Results

Our solution achieved a prediction accuracy of ~90% for heart failure cases, identifying approximately 70% of high risk patients for the onset. The timely early identification enabled clinicians implement personal and intervention plans, potentially averting severe health outcomes. For California, the PoC demonstrated potential cost savings of $5M if preventive measures were applied to a fraction of the high-risk population.

Business Value

As Principal Data Scientist, I lead a team comprising experienced data scientists working with healthcare domain experts, and business strategists to develop a scalable predictive modelling, clinical data interpretation, and healthcare analytics. The engagement helped demonstrate the capability of the Vantage for scalable analytics which contributed to a huge win for Teradata with the sale of Vantage platform to the Payor, the highest reported sale in Teradata’s history.
